BENEFITS SUMMARY
CLINICAL FELLOWS, TRAINEES AND HOUSE STAFF
Health Insurance is available through United HealthCare (UHC). Enrollment must be made within the first 31 days of your appointment with the School of Medicine. You may apply for coverage for yourself and your dependents. Your coverage effective date for health insurance is based on your appointment date. If your appointment date is the first of the month, your insurance coverage will be effective on your appointment date. If your appointment date is the second of the month or later, your insurance coverage will be effective the first day of the following month.
You are responsible for complying with the procedures for your particular plan. Insurance premiums are made by payroll deduction. All Clinical Fellows are encouraged to purchase insurance coverage for themselves and/or their families. You are responsible for all applicable deductible and co-payment amounts.

DENTAL INSURANCE:
Dental Insurance is available through Essex Dental Benefits, a preferred provider organization (PPO). You must enroll within your first 31 days of your appointment, and you may apply for coverage for yourself and your dependents. Your coverage effective date for dental insurance is based on your appointment date. If your appointment date is the first of the month, your insurance coverage will be effective on your appointment date. If your appointment date is the second of the month or later, your insurance coverage will be effective the first day of the following month. Insurance premiums are made by payroll deduction.

LIFE AND AD & D INSURANCE:
Optional term life and accidental death and dismemberment (AD&D) insurance is available through MetLIfe. You may purchase optional coverage equal to from one up to four times your annual stipend. You may also purchase optional dependent coverage of $25,000, $50,000, $75,000 or $100,000 for your spouse, and $4,000 for your dependent children. Premiums are made by payroll deduction.
LONG-TERM DISABILITY:
Clinical Fellows are automatically enrolled upon their appointment with the School of Medicine. This Long-Term Disability plan, insured by The Paul Revere Life Insurance Company, provides a benefit of 60 percent of your monthly stipend, to a maximum benefit of $3,000 per month after an elimination period of 90 days, if you are disabled due to an accident or illness. Premiums are made by payroll deduction.
Effects of Leave on Benefits
When an extended leave of absence is needed beyond the time allotted for vacation or sick leave, house staff members should contact Human Resources for the employing institution to discuss the choice of methods for continuing medical and dental benefits while on a leave of absence.
