It is the responsibility of each department to have the hard drive of their computer "wiped clean" before disposal.

After sending the request to EH&S, please print and attach this form to all pieces of equipment that need to be removed. EH&S staff will not pick up any equipment unless a form is attached.

Request may take up to four (4) weeks for pick-up.
This is a free service, however, EH&S does not have the resources to accommodate disposal of equipment that is heavier than 750 pounds or greater than 6 feet in any direction. It is the responsibility of your department to hire commercial vendors to facilitate removal and disposal of equipment that EH&S cannot accept.
Additional information: The St. Louis city fire code does not allow storage of electronic equipment in corridors on the Washington University School of Medicine campus. Departments are responsible for compliance with storage requirements until EH&S removes the equipment.
